If you have many students in your area, you may complete the &lt;a href="/id090c7f6a8024c439"&gt;&lt;strong&gt;spreadsheet version&lt;/strong&gt;&lt;/a&gt;. &lt;br /&gt;&lt;br /&gt;This form must be completed for all status changes to the student employee. This includes hiring a student or terminating a student. During the year, when a student receives a merit increase, the &lt;a href="/id090c7f6a80234f3b"&gt;&lt;strong&gt;performance review&lt;/strong&gt;&lt;/a&gt; will serve as documentation to increase the wage.
