Authorization to Disclose Information

All students requesting information to be sent out of the health center or if the health center requests that health records be sent to Drake need to complete the Authorization for Release of Medical Information. This form specifies who is releasing the information and where you would like the information to be sent. All substance abuse, mental health and HIV-related information need to be specifically authorized for release of information.
