Immunization Requirements

All new students born after 1956 must provide documentation of receiving 2 measles, mumps, rubella shots (2 MMR's) OR provide documentation  of positive blood (lab) titres for measles, mumps and rubella. Signatures of medical providers or photocopies of medical records are required. Immunization requirement forms are available from the Student Health Center, at summer orientation, or the dean of student's office.

The TB questions on the health form MUST be answered.  If you answer "yes" to any of the questions listed, then a TB (tuberculin test ) MUST be done.  (If a tuberculin test has a positive result, then a chest xray MUST be completed and the chest x ray report must be sent).  You may also do the tuberculin test before you arrive at Drake, or you may the test at the health center at Drake for a fee. If you answer NO, nothing further is needed.

The meningitis vaccine  is highly suggested for new students living in the dorm setting. The state of Iowa requires a signature of waiver if you decline the vaccine.

Students who have not satisfied the above requirements will not be permitted to register for the following semester’s classes.
