You may use the following links to gain assistance. Please note that this is not an additional appeal stage. These procedures do not replace or override policies and procedures of the colleges and units. Student complaints and appeals will be reported to, and tracked by, the Vice Provost's office.

Complaint and Appeal Assistance Form: If you are not sure to what office to submit your concern, complete the Complaint and Appeal Assistance Form. You will be advised about the appropriate office to contact.

Complaint Tracking Form:If you have completed formal complaint or appeal process in a college school or other unit but wish to add more information for the record, please complete the Complaint Tracking Form. Please note that this does not constitute an appeal of any decisions made through an official complaint or appeal process.
